Liberty Dental HMO Plan members that are assigned to a Primary Care Dentist may change their assignment by visiting LIBERTY's mobile app where Members may search, select and make a transfer to a new dental office. Members may also contact Member Services Department at (888)700-1093. Requests received by the 15th day of any month will be effective the first day of the following month.
